American Association of Clinical Endocrinology Consensus Statement: Algorithm for Management of Adults with Dyslipidemia - 2025 Update

The 2025 update to the American Association of Clinical Endocrinology (AACE) dyslipidemia algorithm provides visual guidance (concise algorithms and tables) to aid clinical decision-making for managing adult dyslipidemia. The primary objective is to reduce the risk of Atherosclerotic Cardiovascular Disease (ASCVD) and triglyceride-induced pancreatitis.
An international, multidisciplinary task force convened to update the 2020 algorithm, reaching a consensus reflected in the new visual graphics and narratives.
The comprehensive 2025 algorithm covers 11 key sections, including:
  • Guiding principles for patient-centered management.
  • Risk assessment and testing.
  • Identifying genetic dyslipidemias.
  • Specific lifestyle recommendations.
  • Targeting LDL-C with pharmacologic therapy for ASCVD prevention.
  • Management of statin-associated muscle symptoms.
  • Management of hypertriglyceridemia.
  • Management in special populations (e.g., older adults, transgender care, pregnancy, lactation, autoimmune disorders).
  • A list of FDA-approved medications.
This update aligns with the new 2025 AACE Clinical Practice Guideline and guidance from other international bodies, emphasizing individualized pharmacotherapy, ASCVD risk assessment, and pancreatitis risk, while also considering health equity and cost-effectiveness.

The ESPEN guideline on clinical nutrition in surgery (Update 2025) states that early oral feeding is the preferred mode of nutrition for surgical patients. Nutritional therapy is considered mandatory for any patient at nutritional risk, especially those undergoing upper gastrointestinal surgery, to prevent underfeeding and postoperative complications. The guideline's focus covers the nutritional aspects of the Enhanced Recovery After Surgery (ERAS) concept and the specific needs of patients undergoing major surgery (e.g., for cancer) and those developing severe complications.

The Global Leadership Initiative on Malnutrition (GLIM) introduced an approach for malnutrition diagnosis in 2019 comprised of screening followed by assessment of three phenotypic criteria: weight loss, low BMI, and low muscle mass, and two etiologic criteria: reduced food intake/assimilation, and inflammation/disease burden. This planned update reconsiders the GLIM framework based on published knowledge and experience over the past five years.
